Development of a Falls Care Bundle (please see attached PDF) designed to identify modifiable objective markers for undiagnosed or under managed medical conditions.
Using Falls as a marker the proactive approach of the first point of contact health care professional (eg. ECP APP GP practice nurse) facilitates the commencement of interruption of the ‘Falls Career’ of the patient by identifying modifiable medical risk factors which may coincidently be impacting on the patients propensity to fall or risk of falling. Equally there is an opportunity for ECP’s to flag up potentially unidentified or under managed pre-existing medical conditions requiring action from e.g. the GP.
